New Diver Application - For New Divers to PDC or divers that have been away for more than one year.
REMINDER: For Returning divers, registration with PDC will first require emailing to inform us of your interest to return and to make a schedule request... After receiving your schedule confirmation, mail in the Contract & Registration Form, the Medical Release Form and Fees.
New divers begin by submitting a New Diver Form... FYI: Returning divers in good standing are scheduled first, then openings are offered to new divers.
Remember, PDC will register all divers with AAU this year. We will provide AAU cards for each diver after all forms and fees have been collected and practices have started!
You really need to read this...it might help you avoid paying a late fee!!!
After your schedule has been cofirmed with PDC, you need to complete these two forms...they need to be signed and intialed in all areas to be accepted! You should be able to tab through the fields to complete the information...then print and sign.
