Each of the following 24 statements was designed to help you and your partner learn something about a particular aspect of your lives. The connection between the questions and the subject of fertility is sometimes obvious; at other times the significance of your answers will be more subtle. For this reason, you should both answer these questions spontaneously and then go on to the next, without stopping to think about what your answers mean. The fertility scorecard at the end of the test will help you understand the meanings of your responses as well as providing some important information on the current status of your reproductive health.
To take the test, simply read each statement and then record on a piece of paper how true that statement is for you, indicating A for always, S for sometimes and N for never. Keep a numbered list of your answers.
1) I drink more than 4 cups of coffee a day.
2) I drink more than 2 cans of cola a day.
3) I take pain relievers regularly- more than 30 tablets monthly

To accurately figure out which numbers indicate your correct weight, you must determine if you have large bones or small bones. Use this formula:
- Small bones= a wrist measurement of 5 1/2" or less; ankles 8" or less
- Large bones= a wrist measurement of 6" or more; ankles 9" or more
For small bones, use the lower weight as your guide; for large bones, use the higher weight. (Note: height includes 1" heels; weight includes 3 pounds of clothing)

How Stressed Are You?
One of the most frustrating aspects of stress is that it can be so deceptive. In our busy lives we can get so used to feeling tense, rushed, fatigued and even somewhat depressed that we begin to view tension as a normal state of being! Many people don't realize how much tension is present in their lives until a diagnosis of stress-related infertility is made. For this reason, it is important that you listen to your body for signs of HIDDEN stress that could be affecting your reproductive health. Ask yourself these important questions:
1) Am I tired all the time?
2) Do I have frequent headaches?
3) Do I catch alot of colds, the flu, or sore throats?
4) Am I prone to vaginal infections or herpes?
5) Is my menstrual cycle irregular?
If you answered yes to any of these questions, stress may be a problem. If you said yes three times or more, your reproductive health may already be in jeopardy.
What Is Your Food IQ?
Living in the weight conscious, diet crazed world that we do, it's sometimes easier than we realize to become undernourished. Although your weight is a good barometer of whether or not you are taking in enough calories, calorie counting alone is often not enough to ensure that all your needs are met. To discover if you are undernourished right now answer yes or no to the following questions:
1) Do you eat atleast 45 grams (180 calories) of protein a day?
2) Do you eat 2 servings of vegetables per day?
3) Are you a non-smoker, and do you drink alcohol very rarely?
4) Do you take a pre-natal or super high potency vitamin everyday?
5) Do you eat 2 pieces of fresh fruit everyday?
6) Do you eat atleast 4 foods a day high in calcium?
7) Have you not taken birth control pills for atleast 3 months?
8) Do you rarely eat frozen dinners or processed foods?
If you answered no to even 1 of the above 8 questions, you could be lacking in an important nutrient. If you answered no to 3 or more, it's possible that you may have some severe nutritional depletions.
TO SCORE YOUR FERTILITY LIFESTYLE QUIZ: Total your A's, your S's and your N's, assigning them points as follows: A responses get 10 points each; S responses get 5 points each; and N responses get 2 points each. Add your total score and then use the Fertility Scorecard below to determine the current status of your reproductive health.
FERTILITY SCORECARD
50-100 POINTS: Your fertility is likely in good shape, or atleast your lifestyle habits are not making significant contributions to future problems. It is also likely you are a well-balanced person who does everything in moderation. If you maintain your current lifestyle, it is likely you will preserve your fertility and your sex life for many years to come.
101-150 POINTS: You're not quite as careful as you would like to think! While your reproductive health is probably not in any imminent danger, if you keep on doing what you are doing, or add a few more excesses, you may find your fertility in trouble within the next few years.
151-200 POINTS: You are in the danger zone!!! If you are not having problems conceiving, it is probably because you haven't tried! This high score should tell you that changes need to be made if you want to preserve both your fertility and your sex life in the coming years.
201-250 POINTS: You are in serious danger!!! Not only is your fertility being jeopardized, but your general health is in trouble as well! If you are able to conceive, your chance of miscarriage and/or birth defects is probably higher than average. If you are also over age 30 and have been living this way for some time, you are at especially high risk for infertility or a problem pregnancy.
